Membership

To renew your PBCC Membership:

1. Download and accomplish the Membership Registration Form.
2. Please secure your payment (Cash or cheque) to:
   BPI/PHC with Acct. # 4251-0061-35.
3. Send an email with the following attachments to pbccsecretariat@yahoo.com with the subject PBCC Membership.
   • Accomplished Membership Registration Form
   • Deposit slip / transaction slip of your payment
   • Existing PBCC ID card (if applicable) or valid ID
4. Wait for the PBCC Secretariat to validate your payment. A confirmation email will be sent to your registered email address.
